Abstract

In the beginnings of century XXI the key note for the understanding of the local in the global is the social exclusion, generator of a underclass and uprooted population with and with doubtful projections on its supervivence, phenomenon that at the present time finds endpoints in deadly processes that can be conceptualized of social extinction. In this context the profile of the social policies has been designed on a pattern in which they stand out the privatization, the decentralization and the welfare and focused practices. In the health area these governmental technologies in convergence with the implementation of the deregulation process and labour impoverishment have molded the processes of work in health. Thus, the reconfiguration of !he health system and the new conditions of work actually conform a permanent tension when colliding with the emergency of new and increasing forms of suffering product of the processes of social exclusion-extinction. From this perspective our interest goes, specially in the field of the mental health, to the analysis of the new sufferings product of these worsened macrosocial transformations from the ninety and of the present conditions of work that affect the possibilities and the limits for the intervention and resolution of the demands in the public health. The work is developed in four axes: a) the evolution of the public health in Argentina with the analysis of experiences located in Buenos Aires: b) the configuration and the discussion. about the new social sufferings; c) the conditions of the process of work in which the perspective of the institutional analysis are developed the practices and d) and the perspectives of the institutional and labour analysis of the actors of the sistem of health with the inclusion of the layout of lines that contribute for the innovation in the institutional practices. The analysis of the proposed subject is based on a qualitative methodology; within the framework of the paradigm of the complexity Secondary data were used like sources of intelligence, interviews in depth and participant observation carried out in hospitals and urban zones
